If your insurance company is not listed above, you may still be eligible for “out of network” benefits. Some companies/insurance plans have very good out-of-network benefits, while others offer none at all. Please contact your insurance company or your member benefits booklet to find out what your out-of-network benefits are. Typically we can bill for your out-of-network benefits through our online billing software, but if your insurance company prefers, we can also provide you with a receipt that you can use to file claims and be reimbursed by your insurance company directly.
